1] If your child is having any newly diagnosed/ existing medical condition
· If they are on any regular medication
· If your child is having any known allergy to food or medicines and medicine is prescribed for the same
· if your child has prescribed any EpiPen (Epinephrine Injection) for the allergy
2] Parents are requested to submit a detailed medical report and individualized health care plan for the child from the Consulting Medical practitioner.
3] This has to be strictly followed and the last date for submitting the report is 05/12/2019.
4] This procedure is strictly need to be followed in future also.
5] This email need to be given utmost importance since knowledge regarding the existing health status of the child is necessary while providing any kind of medical care.
