Kindly Note,
1. Required attachments along with this forms are as follows :
· Previous vaccination record
· Letter from parents : (need to submit only if they do not have vaccination card)
Content of the letter should include:
(The reason for not submitting the vaccination card ,whether the student is vaccinated up to which age).
Please note-
1) All forms should be handwritten by the parent with a signature.
2) Parents are requested to submit the hardcopy of all forms on or before 30/04/2026.
