Parents Consent Form (Grade 6-12)

Respected Parents,

                                     Please note the following-

1] Parents as you are aware that ADEK has conducted a survey in the last week of June 2020 regarding Reopening of schools in September 2020.

2] In This regard, we are planning to ensure that students return back to school in September, 2020 as per ADEK Private School Reopening Policies and Guidelines document received on  02.07.2020

3] Please inform us via the attached acknowledgement of  consent of sending your ward to school from 30.08.2020. We expect all the parents of Grade 6-12 to complete the attached word document and email it to the respective class teacher latest by 11.07.2020

4] Please note that your duly filled acknowledgement will assist us in planning and working out the reopening plans more accurately and systematically.

5] We request all the parents of Grade 6-12 to complete the attached word document and email it to the respective class teacher latest by 11.07.2020
